Assessments & Reports

Healthy Nature uses a trauma-informed, person-centered, evidence-based assessment and reporting framework to assist with collating and presenting detailed, specific information on life domains impacted by disability and the corresponding areas of support needed for individuals based on:

  • Capacity
  • Performance
  • Environmental Factors
  • Impairments

We use NDIA-approved independent assessment tools mapped holistically to functional domains underpinned by the NDIS Act 2013 and aligned with the WHO’s International Classification of Functioning Disability and Health (ICF) model.

Our approach aligns with the NDIA goals of “consistency and reliability of access and plan funding decisions to help deliver high quality and appropriate supports” (NDIA, 2020a)

Our assessments and reports utilize NDIA and NDIS-approved structures that assist in the following:

  • Identifying your NDIS goals
  • Identifying early intervention needs
  • Explaining your therapy approach
  • Provide evidence of the outcomes achieved
  • Highlight the progress made towards your goals

Healthy Nature Specialises in NDIS and NDIA independent assessments and reporting, including the following:

  • Functional Behaviour Assessment (FBA’s)
  • Functional Capacity Assessment (FSA’s)
  • Biopsychosocial Assessments
  • Risk Assessments
  • Preliminary assessments for dual diagnosis (disability, mental health, substance misuse)
